Who is "we"? What is the reason for this survey, and what will the results be used for?

Your survey is confusingly constructed. Your third question says something like "If you are not likely to use our service, etc" but if the respondent decides they are likely to use it (meaning it applies to them) it forces them to put an answer, even though the situation doesn't apply.

I hate the choices currently offered and think I could do a better job for the average consumer.
