Calibre TOC

I'm pretty much a newby with Calibre, and I've had mixed results getting a table of contents out of it. Sometimes I can get one and sometimes I can't. With a current document I can tell it to force a TOC, but nothing happens.

(Trying in mobi output.)

Can anyone make a wild guess why this is happening?

I did one starting with a PDF last night that made a TOC. This morning I'm trying a different one but get no TOC.

If there are too many chapters does Calibre just do nothing at all?

Unfortunately PDF is just about the least satisfactory format there is as a source to convert. This PDF sticky attempts to educate you on what you're up against.

The PDF's own TOC will not be used in the conversion, nevertheless, if your PDF chapters are marked in a 'standard' way (e.g. 'Chapter 1', 'Epilogue' plus other common variants) then the Convert - Heuristic Processing page may help you. You will need to read the relevant part of the Calibre manual to decide which checkboxes to check/uncheck for any particular PDF. There's a link included on the Heuristic page.

Actually I've tried PDF and a few others. None of them is consistent with TOC. I'll look at heuristics again. I didn't see anything there that seemed useful.
