I read, and have music going in the background at home, and I have my headphone on when commuting while reading. While I hear the music, I don't HEAR the music. Most of my brain is on the book. Unless a very favorite song comes on, then the music comes a bit more to the front.

Working in a noisy front lobby, I learned how to focus on my work and ignore everything else unless they need my assistance.
