Mobipocket Creator does a good job in removing headers/footers from PDFs, but otherwise the new version of calibre does better conversion of fonts (types/sizes) and formatting. Both have their Pros/Cons - it's best to try both on each document...

Mobipocket:
Pro: auto removing headers/footers, processing of pictures
Cons: missing cover image, formatting glitches (justification errors, font sizes)

calibre:
Pro: good style and formatting conversion
Cons: mirrored/inverted images, removal of headers/footers
