This error message is typically displayed if an NCX TOC contains a broken fragment id. Usually, recreating the NCX TOC will fix this problem. It also couldn't hurt to check your book with the IDPF Validator.
Thanks, Doits. It didn't make sense to me, but...I don't use it.
