I know about modify epub and the polish feature all im curious about is the proper way to do both: Replace and add covers

For example I have a few epubs that have no cover so calibre has assigned its title page as the cover. How do i add the cover on top of that.

Secondly if a book currently has a cover and its rubbish how can i replace it correctly and not just add a cover on top of it.
