The process is simple enough:

* download a bunch of files
* install the disk imager if you're on windows
* image the SD card with disk imager
* copy the CM zip file and the gaps zip file to the SD card
* turn off Nook HD(+)
* insert SD card on Nook HD(+)
* boot device
* install zip and select the CM zip
* install zip and select the gapps zip
* remove SD card
* reboot
* enjoy
