The actual bug was here:
> Python function terminated unexpectedly: 'utf8' codec can't decode byte 0xa0 in position 30866: invalid start byte
I think this is a bug already fixed in very recent versions of calibre. Please update to the very latest version of calibre and try again.

The last line leads me to believe that your original has a encoding error at position 30866 (into the azw3 source file)
(there may be other errors as well: The div table for SKEL0000000004 has incorrect insert positions. Calculating manually. )
