I recommend pdf995 (http://www.pdf995.com/. It acts like a printer driver.
You can use ANY program that lets you print to create your document, click print, select the pdf995 driver and it will print to a .pdf file (you can also set a custom page size so you aren't limited to standard paper dimensions).
Very flexible and free.
