From what I can discern, Monaco Monospace is an old Apple font that has never been open-sourced (I don't think it even ships with a new Apple computer any more). You likely have one of the knockoffs that were based on the original (most certainly if you're using it on Windows). It may just be a matter of tweaking the font's flags with something like FontForge to get Qt to recognize it as a monospaced font.
