There's PDFtk, which has a free distribution for OS X. It's not the most user friendly thing out there, and I've only ever used the Windows version myself, but it's something.

Looks like there's a html form interface for it posted lower on the homepage there, which may be easier than using it command-line-style. Haven't tried that either.
