You're copying the wrong file, or you're copying it in the wrong place (or you're copying it wrong, somehow (i.e., corruption during transfer)).
It's pretty much an atomic decision, which means PEBKAC.
The only other explanation is broken/dying storage, but you'd have suffered through a whole other host of symptoms first.
