It depends on the suffix.

Jr. and Sr. are typically offset by a comma.

II and III are not.

Titles are offset by a comma. MD, Ph.D, etc.

So:

John Smith, Jr.
John Smith, Sr.

John Smith II
John Smith III

John Smith, MD
John Smith, Ph.D
