Spans.
Which might explain why on my vandalistic removal of Drop Caps,
SMALL CAPS and Raised Initials that I've only ever seen the miscreants (like the House of Random Penguins and everyone else) use spans.
It's fine to do it for yourself, if you like that kind of thing.
