Each page element must have a name attribute that indicates a page name and href attribute that indicates a reference to the first piece of content on that page. XPointer can be used to allow referencing down to individual characters. The name attribute is often a number, but can be any string. For the cover, you may want to give it an empty string, so that no “page number” or name shows up next to the cover.
