So far as I can tell, an ebook on Overdrive for libraries is just like a physical book. A library buys a copy and can then lend it to one reader at a time, if they want more concurrent readers they buy multiple copies. One bad part of this approach is that MOBI and ePub (say) copies are separate, if a library wants to have both formats they must buy copies of both.
