The avatar is from a scene in the Season finale of Smallville. Clark Kent had found himself in a position where he could have had to leave Earth permanently. His adopted mother gave him a present before returning to Washington, D.C where she currently works. The note on the package said simply, "Should you decide to stay."
The reflection in his eye is all viewers got to see of the package's contents. The assumption is that she finally made the traditional costume for him and left it as a gift.
This episode also marked the first time in the 9-year series where, via a vision Clark (now a young man) was given of the near future, Superman was seen in full costume flying over Metropolis.
Surely even in Italy you folks know about
Superman. He's a world traveler, you know!
