There is no real short anwser to why the cover doesn't show. Does the cover show in Calibre or ADE? If the cover doesn't show there, it is probably the epub. If it does show, then probably IPad is at fault.
If you could post the epub, or parts of it, we can take a look at it. What method do you use for your covers?
