You skipped the conversion to PDB.

What we are trying to do is force Calibre to use certain methods that produce the
result we want.

It is important to have Calibre first import using the HTML decoder then create a
PDB formated ebook, then import the PDB file (using the pdb decoder) and then
create the epub, with the Jbook 5-inch output preference set.

It only takes a few minuets.
