I don't think the story was intended to be terribly original. I've heard this film described as Scorcese "getting on his film geek." For anyone who's seen The Cabinet of Dr. Caligari, Vertigo, Memento, etc., the twist was pretty well-telegraphed. What makes the film worth watching is the execution and the acting.
