You know, my brother-in-law (in Pennsylvania) told my husband the same thing. He said it asked a lot of personal information about banking and credit card accounts and such, too.
I told him that it
couldn't be the government census, but he insisted it was . . . until he got the real one. Now he says that it was something else, yet he didn't say what. Can't imagine what it really was, unless it was a scam.
