Nah, you can have have root folders/files, it should matter.

Very strange. Is PRSPInstaller folder visible via Browse Folders? Does it contain all necessary subfolders as mentioned in post #707? If yes, sounds like disabled SD/MS card scanning, but you say you've enabled it...

Anyway, extract the attached file to your /database/system/PRSPlus/addons folder on internal memory. Then go to Games & Utilities menu, prs+ installer should appear there as an option.

PS
If it stucks instead of opening the PRS+ Installer, don't worry, just connect reader to USB port, if that doesn't help, reset, the problem will be gone.
