I've checked a number of my books, and none have the problem. I create TOCs in two different ways:
1. For straightforward books I use the "Insert TOC" menu command.
2. For more complex ones I do it by hand with note/link
Both of these work fine for me. Perhaps it's changing the style of the TOC which is the cause of the trouble?
