On Mobipocket FW? It never happens to me but I don't add the covers with Calibre. I had some problems with it, so I add the cover with Mobipocket Creator and they always show well.

[Edit] I hadn't seen the last answer. Another thing I never do is sending the same book I've bought, adding only the cover. I strip DRM, obtain the OEB directoy, change the things I don't like (typically adapt format, change cover and add description and series in Publisher field) and I generate the new PRC. In this way I never have had any problem with the cover.
