I'm thinking there may be a formatting (or some such) error in the book that is showing up only on the reader and the built in Library viewer handles it okay. ...

Which Sony Reader do you have?

Have you contacted Sony? If not maybe an email to support?

Maybe someone else can either confirm or deny?
