The worst offender I've seen had six copies of the cover, most of them just images of a plain text cover. As near as I can tell, the intent was to foil Amazon's ability to send out a sample that contained any actual text.
At least if you are stripping them out, you can just remove the extra images (I'd probably dump both and let Calibre add the cover) and rebuild it.
