Yes, bugs happen, I create my own fair share, but this bug was discovered within minutes of the upgrade (and by others before me) - it was obvious and easy to find. I reiterate, if using a proper testing strategy and test scripts this will not happen. Generally issues like this only make it to production when testing is limited and/or developers are doing their own testing.
