I'd say it's a "bug" in the reader. There is no way (that I know of) in ePUB to specify top and bottom margins. You can only set margins in the non-standard Adobe-specific page templates. Even for right and left margins, I strongly believe they should be settable by the user in the reader software, and not in the ePUB book itself.

There is also no way to use the screen size (particularly screen height) in setting the sizes or positions of images (or any other element). This is probably the reason behind other problems you see.
