Not all transformers are created equal. Anything converting wall power for electronics runs power through coils via induction. This effect also cannot go on forever. Eventually the coating on the wires in the coils wears out, shorts develop, and the transformer itself fails.

I've used stuff like this in the past on my Sony Reader, haven't had any burn out on me yet. I'd chalk this up to it being 'time' for that adapter to just die. A transformer can really only fail while current is applied, so it isn't uncommon for it to do it shortly after being plugged in if it was already close to dying.

That said, if this was brand new, it was likely defective.
