This occasionally happens to me, although most of the time it comes out fine even if the chapter heading is a separate block (as long as Abbyy knows it's text, and if it doesn't, you can right-click on it and change it to text). My gut feeling is that Word tries to call a separate block a header when it is too close to the top of the page.

In those cases I generally just type the missing chapter heading into the Word file. There are probably more elegant ways, but since on most books it doesn't happen that much, for me it's always been easier to just fix it than to figure out a way to prevent it.
