Use EPUB as the source for conversion whenever possible. PDF input is rather limited and does not produce the best results a lot of times. It is being worked on but it will never be as good as EPUB. One major reason is EPUB is a reflowable format so it's very easy to tell where one paragraph begins and ends. PDF is a set layout. While there are ways to determine paragraphs they are not 100% and can / do detect incorrectly. Oh and easy stripping of headers and footers in PDF files is not available yet. It is possible but a bit cumbersome at the moment.
