Still don't really understand what's causing the TOC to be so utterly screwed up, but I found a workaround to all the problems... primarily by using unnumbered and unnamed (and thus invisible) sections to get the given TOC entries to link to the correct page (despite technically incorrectly linking to said invisible sections).
