Mine didn't initially either - but I poked the reset button in the back, and then the reader happily restarted from scratch, read the card, and went into the Hack Builder menu.
Once I'd told the menu to install the hacks ('5' key) my reader sat around for about 10 mins, telling me it was rebooting. I waited as long as I could and then cautiously pushed the '6' key (the next stage in the process.) A message popped up about "this shouldn't have taken long, there may be an problem" but then it reset, and everything had installed correctly! No books / bookmarks lost, no problems found. (I do need to reload some books, which I had been reading: my new font gives me more text on a page, and so the old pagination isn't going to work.)
My Reader has had all the Sony updates, so ... for anyone thinking of going for this but put off by the Hack/Flash software being 2 years old ... Don't Panic! I have joystick page turning, small-fonts, amended default font (oh how nice that is!) and the new icon set (so pretty!) It's all good -- worth the temporary "Oh My Gosh, I'm going to permabrick it" fear.
