Which software are you using to create your Mobipocket book? I use Mobipocket Creator, Publisher Edition. Currently have v4.2, but haven't looked lately to see if there's a newer one.
Our own Harry T has written a quick
tutorial on how to use Mobipocket Creator. His hints/tips on creating the TOC may be very helpful.
