If the blood clot dries out or falls out, it can apparently lead to swelling and excruciating pain in the exposed bony bits. I have a vague recollection that it happens mostly to the lower wisdom teeth and moreso if the tooth was impacted (or, as that's why I had mine removed, it might just have been the dentist trying to scare me)
