The easiest solution is to load the file into "Word" and then save as RTF. Select the "File/Properties" menu item to enter the document title and author's name.

There's a very nice Word macro posted here by Stingo for nicely formatted a text file in Word - getting rid of the line breaks, changing to a decent sized font, etc. Well worth using.
