I picked up a SanDisk ImageMate 5-in-1 (SDDR-99). The key is to make a DOS_FAT32 partition table with a FAT16 filesystem. Windows blue screened with USB OHCI every time when I tried to format the card in explorer, so I did it on linux with

mkdosfs -F 16 -n "sonycard" -v /dev/sd?1

I was able to restore my PNY 2GB SD card back to working state(now properly shows up in CONNECT).
