You're saving the metadata, including the cover, in calibre's files. The cover is not "sticking" to the book because that would require altering the book's files. Calibre does not alter your books without you explicitly telling it to do so - which is why you need to do the conversion.

Actually when you save to disk, calibre will update metadata in the saved copy. However, setting covers in EPUB files is hit and miss, since the EPUB format doesn't support covers.
