I must admit that I did most of my horror reading awhile back, and haven't been reading much in the way of horror the last couple of years, but I thought The girl Next Door was a chilling read. It's so brutal and cold with absolutely no redeeming characters. I don't think Ketchum is as extreme as those who followed after him, but then again, I haven't read much in the way of extreme/gory horror in a while.

I'm not much of an "extreme" horror fan (most books billed as such seem to try too hard and accomplish too little) but that wasn't the source of my disappointment with
Off Season - I just thought it was a lackluster book in general, regardless of genre or intent. I've heard great things about Ketchum's writing, though, so I'll give his work another shot at some point.
