Of course, I accept your statement about a bug having been present. But I'm not sure about your conclusion about the unique trigger.
I've just checked my three different machines and they all have ' "github": true; '
I've also checked previous versions of the EpubCheck settings preferences (EpubCheck.json.old) and it looks like I've had github set to 'true' for ever.
But that strange version name 'v5.4.0-fix' never seemed to impact your plugin's performance on my linux, W11 or MacBook - it identified errors which needed fixing.
And I typically use your plugin many, many times during a work session.
