But what is triggering this removal in many xhtml files if only the two xhtml files are being edited?

But OP has specifically said only three files were edited by him: 2 xhtml and 1 image file. So what triggered the toc.ncx rebuild if he didn't? His specific edits certainly wouldn't have warranted a manual ncx rebuild.
