the font used gave me the clue: Velum was used to code that mess.
Velum uses multiple H# tags in their heading blocks. (an old school way of styling in the 80's, before CSS)
Calibre also breaks on H# tags by default (preferences (or the start of conversion): common options: Structure detection)
In this respect, I agree with Jon, Vellum writes garbage code. It works as delivered, but trips up 'clic here' type conversions.

If I need a second line heading (AKA title) I usually code it all as 1 heading.

Another advantage ir the TOC tool make the entry: 123 One or Two or Three

I have a bunch of saved searches (i use as templates) to adjust what I deem as excessive waste (12em minimum) between the chapnum and the chaptitle that velum uses.
I use 6em, not just nuke all spacing
