Still no luck.
1. I rebuilt an image from scratch, going back to the B&N template, then Paint Shop Pro to crop the front page from the entire cover, then ran through the entire process. Tried varying resolutions up to 600 just as a test. No improvement.
2. Reviewed all the settings in Calibre that seemed to be relevant, no improvement
3. downloaded and installed Modify Epub, thought this would give me direct access to the cover file details in the epub, as the Edit book in Calibre does not display the front cover image title detail, at least not anywhere I can find. However. Modify Epub does not either (at least not directly). There is a checkbox entitled "Insert or replace cover" that did not give direct access, only something 'mysterious' behind the scenes was done that had no effect. Might be misusing the plugin, but end result is no improvement.
4. I've gone through preferences in Calibre again, might be missing something but no improvement.

The cover looks great up through Calibre View option, AND the .jpg that is in the output folder along with the epub looks fine. But when I click on the generated epub to display in Adobe Digital it is hashed (see attached).

My only conclusion is there is some sort of conversion 'gotcha' within Adobe Digital Editions 4.5.12 and there are no setting preferences that I can find to tweak it.

Open to suggestions as I've run out of ideas.
