I had something very similar to this happening with a book on my Kobo Touch a few months ago.
It opened at the cover image but the page count below was double what it should be and would show the book as being halfway through, even though it hadn't been read.
Didn't matter what I changed, it kept doing the same thing. I eventually had to show it the error of its ways by setting Sigil on it. Everything went in the bin apart from the basic HTML pages, the stylesheet and the images and I rebuilt it from the ground up.
Bit of a nuisance, but sometimes it's the easiest way.
