I'm aware this is an old thread, but I thought I'd share :
Fusioncore's solution worked perfectly, with KobotouchExtended 3.7.2. Unfortunately this version had bugs, notably
this one , (no space before or after italicised words) and it was really bothering me.
So I modified KoboTouchExtended 3.7.3 the way described and it seems to work so far.
I run on Mac OS Monterey, Calibre 6.29 with a Kobo Clara BW. I can't test it with other equipment, but I'm attaching it in case it can be useful to someone else or for further testing.
