Quality Check not "change" the folder structure (which would mean that you could choose it), it fix it. The resulting folder structure will always be the one of Calibre "author/title (calibre_id)/" but the Quality Check action will fix it the if the current real path used is not the good one.

Normally, when metadata are changed, Calibre automatically change the path of the folder structure to match them. But in rare cases, it failed to change the folder path wich lead to a invalid folder structure (this happens mainly on Windows). By the way of how Calibre work, such case don't raise issue because Calibre still locate the files.
