The last time I had one 'just fail' was spectacular. I had a 5.25 drive in a desktop caddy and all of a sudden it started speeding up and was literally bouncing around on my desk before I got it turned off. It was my backup drive that had seen prior service.
In all other cases, they started making some sort of errors (timeout or bad blocks)
