This suggests an alignment issue with the IR touch. You might want to check the following thread: kobo aura hd touchscreen suddenly stopped working. You may also want to search for other posts by Ken Maltby.

Given the Aura HD boots to the setup screen, the image is the right one and has been installed correctly.
