Download the koreader-kindle-legacy zip file from here: https://github.com/koreader/koreader/releases/latest
Unzip it to your Kindle's root directory, and then KOReader should appear in KUAL.
KOReader runs pretty slow on Kindle DX. To speed it up a little, I suggest starting it in "no framework" mode. Also, disabling all the plugins can help. To do that, look for the "plugin management" section in the menu after KOReader starts.

I tried that but still didn't work, still says no extensions found and what a MESS it made of the root directory. It is hard to tell what goes with what now. That could have been done better I would think by having all of those files in a separate directory. I am certainly doing something wrong but can't quite figure it out yet.
