I tried your instructions (see my test case) and I could not see any issue. BeckyEbook also tried my simple testcase and it worked on Windows. She then pointed out other "inconsistencies" but they are all expected behaviour and have existed in Sigil for many many releases.
She says she can recreate your issue but no one yet has provided a epub test case that shows it since my simple test case does not show it on any platform.
